adverse effect, adverse event, adverse reactionとside effectの違いだが、9版から継続して記載されている。side effectは副次的な作用であり、”beneficial”または”harmful”のどちらの場合もありえる点がadverse XXXと異なる。
11版では新たに”beneficial side effect”という言葉を使った例文が追加された。以下に記載するICHと異なり、AMA Manualではside effectという言葉を使わないようにとは言っていない。
PMDAのサイトでICHの記述（ICH-E2 臨床上の安全性）を見てみよう。
まず、Adverse Eventの記述とその訳文である有害事象 (Adverse Event (or Experience)) の記述を読み比べてみる。
1. Adverse Event
Any untoward medical occurrence in a patient or clinical investigation subject administered a pharmaceutical product and which does not necessarily have to have a causal relationship with this treatment.
An adverse event (AE) can therefore be any unfavourable and unintended sign (including an abnormal laboratory finding, for example), symptom, or disease temporally associated with the use of a medicinal product, whether or not considered related to the medicinal product.

(1) 有害事象（Adverse Event（or Experience））
医薬品が投与された患者または被験者に生じたあらゆる好ましくない医療上のできごと。必ずしも当該医薬品の投与との因果関係が明らかなもののみを示すものではない。
つまり有害事象とは，医薬品が投与された際に起こる，あらゆる好ましくない，あるいは意図しない徴侯（臨床検査値の異常を含む），症状，または病気のことであり，当該医薬品との因果関係の有無は問わない。

E2Aの英語の原文には、上記の記述の後に、”side effect”は使わないように言及している。
The old term “side effect” has been used in various ways in the past, usually to describe negative (unfavourable) effects, but also positive (favourable) effects. It is recommended that this term no longer be used and particularly should not be regarded as synonymous with adverse event or adverse reaction.

次は、Adverse Drug Reaction (ADR) の記述とその訳文である副作用（Adverse Drug Reaction）の記述を読み比べてみる。
2. Adverse Drug Reaction (ADR)
In the pre-approval clinical experience with a new medicinal product or its new usages, particularly as the therapeutic dose(s) may not be established: all noxious and unintended responses to a medicinal product related to any dose should be considered adverse drug reactions.
The phrase “”responses to a medicinal products”” means that a causal relationship between a medicinal product and an adverse event is at least a reasonable possibility, i.e., the relationship cannot be ruled out.

(2) 副作用（Adverse Drug Reaction）
病気の予防，診断もしくは治療，または生理機能を変える目的で投与された（投与量にかかわらない）医薬品に対する反応のうち，有害で意図しないもの。
医薬品に対する反応とは，有害事象のうち当該医薬品との因果関係が否定できないものを言う。

ICHでの用語の定義を認識しておくことは必須。
side effect = 副作用と連想する人が多いと思われるため、adverse drug reactionの日本語訳が「副作用」というのは違和感があるかもしれない。私も昔はその一人だった。
基本的に副作用は体に良くない作用を指すことが多い。医療関係でそうでない説明をしているサイトもあるが。
ちなみに、ワクチンの場合は「副反応」と呼ばれる。
最後に、New England Journal of Medicine (NEJM) にも触れておく。
NEJMでは”side effect“は結構使われており、「副作用」と訳されている。
一方、”adverse reaction“は「有害反応」と訳されていることが多い。
“adverse drug reaction”は”adverse reaction”と比較して、原文で使われている数も少なく、日本語訳を見つけることはできなかった。
